Working tension and biological aging of surgeons under round-the-clock duties

Introduction. Surgeons are personally responsible for the life, health and safety of the most critically ill patients. The situation is aggravated by night and extended hours of work, when, due to circadian biorhythms, the nighttime performance of the human body is reduced and, due to the patterns of fatigue development, performance significantly and steadily decreases after 8 hours of continuous work. According to the current hygienic classification of labour, the tension of labour of surgeons working on round-the-clock duties is assessed according to the highest degree of harmfulness (class 3.3). The processes of wear and tear of the body are most fully described by methods of age physiology.

The purpose is to study the differences in the development of working tension in surgeons under round-the-clock duties in connection with the indicators of biological aging.

Materials and research methods. 59 surgeons aged 23-64 years (M±m: 40.8±1.4) were examined using two methods for assessing of biological aging - the method of V.P. Voitenko (1991) and A.G. Gorelkin and B.B. Pinkhasov (2010). The following were measured: height, body weight, waist and hips, blood pressure, heart rate; samples of Uemura and Stanhe, a questionnaire survey were carried out. Doctors assessed their work tension per shift at the end of the daily duties on a 5-point scale. The elements that make up the production load and form the working tension of surgeons were studied. The data were processed at the level of statistical significance p<0.05.

Research results. An increase in working tension is accompanied by an increase in the frequency of complaints about well-being, in particular, dizziness (r=-0.34, p<0.01), meteosensitivity (r=-0.36, p<0.006), pain in the heart (r=-0.27, p<0.05), shortness of breath (p<0.03); increase in indices of self-assessment of health risk (r=0.29, p<0.03) and risk of chronic diseases (p<0.03). An increase in working tension to a level above the average is accompanied by an increase in the biological age (p<0.03) and aging rate (p<0.05) of surgeons at the end of the round-the-clock duty and an increase in the growth of these indicators over the period of duty (p<0.01), determined by according to the indicators of the functioning of the bloodcirculatory system and complaints about well-being (according to V.P. Voitenko). The average shift increase in systolic blood pressure gradually changes from negative to positive (p<0.01). From the side of a number of indicators - static balancing, reflecting the intensity of blood supply to the brain, and anthropometric indicators that characterize the intensity of metabolic processes and determine the biological age and aging rate coefficient according to A.G. Gorelkin-B.B. Pinkhasov - a parabolic dependence on an increase in working tension was revealed with the most "stimulating" values at its average level. The number of patients that the doctor sees in the wards from surgery to discharge from the hospital was identified as a statistically significant factor in the formation of the working tension of doctors (r=0.27, p<0.04).

Conclusions. An increase in working tension is accompanied by an increase in the physiological cost of performing activities with an uneven load on different physiological systems and organs of surgeons. A number of parameters demonstrate a linear dependence on the tension level, another number of parameters show a parabolic one, certain parameters do not reveal a statistically significant (p<0.05) dependence. In general, the working tension above the average level is accompanied by unfavourable changes in the physiological maintenance of professional activity and the well-being of surgeons under round-the-clock duties, contributing to their accelerated biological aging.
